Here you will find any documents you need to fill out for your athlete, or reference material to help you become more familiar with WAT.  You will need to have Microsoft office software to fill out the interactive forms for sending back to us.  Most of the reference material can be found elseware on this site, but is in pdf format that you can save to your hard drive in case you do not always have internet access.

If you do not have office, there is a free alternative that can be found at www.openoffice.org  If you need help setting up this software to read/write microsoft content, contact Nick Harmsen
